Governance readiness needs a controls-to-operations mapping

A governance layer is the set of controls that defines approved data use, review thresholds, escalation paths, accountability, and traceability for AI-supported work. ISO/IEC 42001 positions AI management systems as an interrelated set of elements intended to establish policies/objectives and processes for responsible development, provision, or use of AI systems—i.e., a management system that can be audited against objectives. Trade-offs and failure modes when agents “skip the decision spine”

Governance-ready orchestration has trade-offs. The most common failure mode is decision drift: as agent workflows evolve, the orchestration layer updates faster than the decision architecture and context systems, leading to unverifiable outputs. A second failure mode is context dilution: you log prompts but not primary records (policy version, scope, risk tier, evidence artifacts), so the audit trail can’t be reconstructed.
